If you want certificate faxed to the vet, write
on the application the name of the office, date/time of appointment
and the pet's name.
Each certificate
includes:
examination, surgery, anesthesia and stitch
removal if necessary.
There may be
additional fee for *dogs over 30lbs for more anesthesia. Retained
testicles, pets over 5 years old, blood test, female cats/dogs
in heat or any other medical procedure not related to the
spay/neuter surgery is not included.
Shots are
mandatory. Vaccinations, antibiotics and pain management
are not included with the
certificate. Ask about shots fees and any other additional
charges when making appointment.
The
recommended age for cats/dogs to be spay/neuter is between 4-6
months. Check with vet.
Certificates are mailed the same day payment is
received.
When you receive the
certificate, call a vet from Muffin's vet list and mention that you
have a Muffin's certificate.
Bring the certificate with you the day
of surgery.
*To have a certificate faxed to
vet
(same day):
*Check the vet
list to see which office accepts a faxed copy of Muffin's
certificate. Make an appointment with vet first, then
charge and request certificate faxed to the vet. After the credit card information is processed,
scroll down to COMMENT BOX. Please add
vet's office, pet's
name, date/time of appointment in this
box.
The certificate can not be faxed
by pet owner, only by Muffin's.
REFUND CERTIFICATE INFORMATION:
There is a $10 service fee per certificate refunded, for the
first six months. Please return certificate with a
self-addressed stamped-envelope. On the front of the
certificate write REFUND with your name and phone
number.
UPDATE CERTIFICATE:
There is a $5.00 service fee per certificate, for the first six
months from date of issue. Please return certificate with a
self-addressed stamped-envelope. On the front of the
certificate write REFUND with your name and phone number.
When Muffin's is
closed, emails are checked throughout the day. Orders are processed,
mailed or faxed the same day. If you have any questions,
